{"id":5443,"date":"2023-04-19T09:07:27","date_gmt":"2023-04-19T09:07:27","guid":{"rendered":"https:\/\/neuropediatoolbox.org\/?p=5443"},"modified":"2026-06-22T16:27:29","modified_gmt":"2026-06-22T16:27:29","slug":"informe-neuropediatrico","status":"publish","type":"post","link":"https:\/\/neuropediatoolkit.org\/en\/informe-neuropediatrico\/","title":{"rendered":"Report neuropedi\u00e1trico."},"content":{"rendered":"<p>There are usually several types of clinical documents that can be issued by a hospital center: <\/p>\n\n\n\n<div id=\"wp-block-themeisle-blocks-accordion-1134d800\" class=\"wp-block-themeisle-blocks-accordion exclusive\">\n<details class=\"wp-block-themeisle-blocks-accordion-item\"><summary class=\"wp-block-themeisle-blocks-accordion-item__title\"><div>History report.<\/div><\/summary><div class=\"wp-block-themeisle-blocks-accordion-item__content\">\n<p>It is the report that collects the information obtained in the first visit. The first neuropediatric visit lasts 30 minutes and consists of several parts:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><a href=\"http:\/\/neuropediatoolkit.org\/en\/historia-clinica-neuropediatrica-plantilla\/\">Medical record.<\/a><\/li>\n\n\n\n<li>Current illness. It consists of the description of the reason for consultation and the systematic assessment of the symptoms that motivated the assessment, in the child's current context (<a href=\"http:\/\/neuropediatoolkit.org\/en\/genograma\/\">familiar<\/a>, <a href=\"http:\/\/neuropediatoolkit.org\/en\/valoracion-cognitiva-del-nino\/\">school<\/a> and of <a href=\"http:\/\/neuropediatoolkit.org\/en\/valoracion-del-neurodesarrollo\/\">neurodevelopment<\/a>), as well as information on other biological aspects that may be relevant, such as <a href=\"http:\/\/neuropediatoolkit.org\/en\/desarrollo-del-sueno\/\">dream<\/a> or food. <\/li>\n\n\n\n<li><a href=\"http:\/\/neuropediatoolkit.org\/en\/pedineurologic-exam\/\">Clinical examination.<\/a><\/li>\n\n\n\n<li><a href=\"https:\/\/neuropediatoolkit.org\/en\/category\/exploraciones-complementarias\/\">Complementary tests.<\/a> They are all those examinations (radiological, analytical, genetic, psychological) that support the diagnostic process. <\/li>\n\n\n\n<li><a href=\"http:\/\/neuropediatoolkit.org\/en\/codificaciones-diagnosticas\/\">Diagnosis.<\/a> Usually, during the first visit, a suspected diagnosis (ID: diagnostic impression) is proposed, or a differential diagnosis is proposed, which will have to be specified in subsequent visits. <\/li>\n\n\n\n<li>Plan. A therapeutic diagnostic plan is established. <\/li>\n<\/ul>\n<\/div><\/details>\n\n\n\n<details class=\"wp-block-themeisle-blocks-accordion-item\"><summary class=\"wp-block-themeisle-blocks-accordion-item__title\"><div>Progress note report. <\/div><\/summary><div class=\"wp-block-themeisle-blocks-accordion-item__content\">\n<p>It is the report that collects the information from the follow-up visit, which usually consists of a 15-minute interview in which several aspects are explored:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Current problems. All those clinical circumstances on which active work is being carried out during the visit.<\/li>\n\n\n\n<li><a href=\"https:\/\/neuropediatoolkit.org\/en\/category\/exploraciones-complementarias\/\">Complementary tests.<\/a> This section refers to the results of pending tests and collects relevant data for the current problem.<\/li>\n\n\n\n<li><a href=\"http:\/\/neuropediatoolkit.org\/en\/codificaciones-diagnosticas\/\">Diagnosis.<\/a> The diagnosis can be updated in successive visits based on the information obtained in complementary tests, the response to treatment, or new symptoms reported by the family.<\/li>\n\n\n\n<li>Plan.
