{"id":7286,"date":"2024-02-17T10:13:15","date_gmt":"2024-02-17T10:13:15","guid":{"rendered":"https:\/\/neuropediatoolkit.org\/?p=7286"},"modified":"2026-06-22T20:07:03","modified_gmt":"2026-06-22T20:07:03","slug":"exoma-clinico-first-tier","status":"publish","type":"post","link":"https:\/\/neuropediatoolkit.org\/en\/exoma-clinico-first-tier\/","title":{"rendered":"Clinical exome, first tier?"},"content":{"rendered":"<p>Technological evolution in genomics has a very fast speed and clinical guidelines become obsolete with respect to usual clinical practice, so that professionals frequently find themselves with questions that are difficult to answer. <\/p>\n\n\n\n<p>Much has happened since the 2003 AAN guidelines, in which the use of karyotyping and fragile X, currently withdrawn, was proposed as the first step in the study of global developmental delay and autism spectrum disorders. <span class=\"zp-InText-zp-ID--19955111-USAKSNND--wp7286 zp-InText-Citation loading\" rel=\"{ 'pages': 'np', 'items': '{19955111:USAKSNND}', 'format': '(%a%, %d%, %p%)', 'brackets': '', 'etal': '', 'separator': '', 'and': '' }\"><\/span>, after the review carried out in 2011 by the same authors <span class=\"zp-InText-zp-ID--19955111-W37ZUI82--wp7286 zp-InText-Citation loading\" rel=\"{ 'pages': 'np', 'items': '{19955111:W37ZUI82}', 'format': '(%a%, %d%, %p%)', 'brackets': '', 'etal': '', 'separator': '', 'and': '' }\"><\/span>.<\/p>\n\n\n\n<p>The 2012 guidelines of the American Academy of Pediatrics introduced for the first time the systematic use of aCGH. <span class=\"zp-InText-zp-ID--19955111-KP9ZKQPJ--wp7286 zp-InText-Citation loading\" rel=\"{ 'pages': 'np', 'items': '{19955111:KP9ZKQPJ}', 'format': '(%a%, %d%, %p%)', 'brackets': '', 'etal': '', 'separator': '', 'and': '' }\"><\/span>. A not unexpected consequence has been the progressive decline of traditional cytogenetic techniques (karyotyping), to the point that the \"know how\" is being lost, and the number of professionals capable of performing the technique is at risk of extinction.<\/p>\n\n\n\n<p>Since then, the appearance of exome sequencing studies has meant a revolution from the point of view of increasing diagnostic yield.
