Neurological examination of more than 3 years:

Main features:

  • From 3 years of age, the neurological examination is almost superimposable on that of the adult, although obtaining information through indirect means remains useful, such as drawing or play.

GENERAL PEDIATRIC EXAMINATION:

  • Auscultation: Rhythmic heart sounds without murmurs or rubs. Good bilateral air entry without pathological sounds.  
  • Abdomen: Soft, depressible, without masses or visceromegaly.  
  • Normoformed genitals of —. 

MORPHOLOGICAL EXAMINATION:

  • Cranial morphology –. Sutures –. 
  • Dysmorphic features –.

NEUROLOGICAL EXAMINATION:   

  • Alertness and higher functions: GCS: 15. Eye opening 5. Verbal response 5. Motor response 5. Language preserved (names, repeats, fluent, understands). Oriented. Attentive. Age-appropriate behavior.  
  • Laterality: Eye: –. Hand: –. Foot: –. 
  • Cranial nerves: Normal confrontation visual field. Normal visual acuity. Fundus of eye/red reflex normal. Pupils equal round reactive to light. Ocular pursuit and EOM –. Hirschberg –. Cover-uncover –. Remaining cranial nerves –.
  • Motor system:  Strength 5/5 in upper and lower extremities. Muscle stretch reflexes present and symmetrical. Normal muscle tone. Normal movements. Bilateral flexor CPR.

  • Coordination: Normal finger-to-nose maneuver. Normal heel-to-shin. Normal Steward-Holmes. Normal gait.
